Test Code PBNP PRO B-TYPE NATRIURETIC PEPTIDE [LAB2670]
Test Method
Chemiluminescent Immunoassay; Roche cobas e systems
Performance
Testing is performed Monday through Sunday.
Routine orders: Typically completed the same day after specimen arrives at the laboratory.
STAT orders: Typically completed within one hour after the specimen arrives at the testing site.
Specimen Requirement
- Lithium heparin plasma is the specimen of choice. 500μL.
- Serum and EDTA plasma are also acceptable sample types.
- Gel tubes are acceptable.
- proBNP specimens are stable for 3 days at 20-25°C, 6 days at 2-8°C, and 24 months at -20°C.
Useful For
This assay is used as an aid in the diagnosis of individuals suspected of having congestive heart failure. The test is further indicated for the risk stratification of patients with acute coronary syndrome and congestive heart failure. The test may also serve as an aid in the assessment of increased risk of cardiovascular events and mortality in patients at risk for heart failure who have stable coronary artery disease.
Reference Values
<300 pg/mLUnder 50 years of age:
NT-proBNP values below 300 pg/mL have a 98% negative predictive value for excluding acute congestive heart failure (CHF). For adults under 50 years of age with intact renal function, NT-proBNP values greater than 450 pg/mL have a positive predictive value of 76% for the diagnosis of CHF. For patients with an eGFR below 60, a cutoff of 1,200 pg/mL yields a diagnostic sensitivity and specificity of 89% and 72% for acute CHF.
50 to 75 years of age:
NT-proBNP values below 300 pg/mL have a 98% negative predictive value for excluding acute CHF. For adults 50 to 75 years of age with intact renal function, NT-proBNP values greater than 900 pg/mL have a positive predictive value of 83% for the diagnosis of CHF. For patients with an eGFR below 60, a cutoff of 1,200 pg/mL yields a diagnostic sensitivity and specificity of 89% and 72% for acute CHF.
Over 75 years of age:
NT-proBNP values below 300 pg/mL have a 98% negative predictive value for excluding acute CHF. For adults over 75 years of age with intact renal function, NT-proBNP values greater than 1800 pg/mL have a positive predictive value of 92% for the diagnosis of CHF. For patients with an eGFR below 60, a cutoff of 1,200 pg/mL yields a diagnostic sensitivity and specificity of 89% and 72% for acute CHF.
